Chronic disease management in Rochester
Living with an ongoing condition often means balancing treatment, symptoms, appointments, and everyday responsibilities. You deserve a physician who takes time to understand how these pieces fit together.
At Rochester HealthSpan Medicine, we review your medical history, current treatment, relevant test results, and what matters most to you. Together, we set practical goals, monitor your progress, and adjust care when your needs change.
Our membership-based primary care model supports continuity for adults throughout the Rochester area. We combine medical treatment with evidence-based lifestyle guidance and coordinate specialty care when appropriate.

Personalized support for ongoing conditions
The scope of care depends on your diagnosis, treatment needs, and whether specialist involvement is required.
Diabetes & metabolic health
Review blood sugar trends, medications, nutrition, activity, and appropriate monitoring. We help you understand your care plan and coordinate recommended diabetes-related follow-up.
Blood pressure & cholesterol
Set individualized goals, review home readings when appropriate, and consider lifestyle changes and medication together. Cardiovascular risk is assessed in the context of your overall health.
Thyroid & other endocrine concerns
Discuss symptoms, existing diagnoses, medication use, and clinically appropriate testing. Endocrinology referral is coordinated when a condition needs more specialized care.
Respiratory & digestive concerns
Evaluate ongoing symptoms and review treatment for conditions within primary care scope. Further testing or specialty care is arranged according to your individual needs.
Medication review & care coordination
Keep your treatment plan clear by reviewing prescriptions, side effects, and recommendations from other clinicians. We help identify questions and coordinate next steps.
Lifestyle & prevention
Build sustainable routines around nutrition, movement, sleep, and stress while keeping appropriate vaccinations, screenings, and preventive care in view.
Improvement without promises of a cure
Some conditions can improve substantially with appropriate treatment and lifestyle changes. Others require lifelong management. We focus on symptoms, function, relevant health measures, and reducing the risk of complications.
We do not promise to reverse every chronic disease. Medication can remain an important part of your care, and changes are made only after individual medical review. Specialist treatment continues when needed.
Our lifestyle medicine, weight management, and preventative care services support a coordinated approach to your health.

Monitor with purpose
Choose appropriate visits, tests, or home measurements that can help guide decisions.
Review & adapt
Discuss response, side effects, and new concerns, with referrals and adjustments when needed.
Chronic care, access & membership questions
Is chronic disease management the same as preventive care?
They overlap, but they are different. Preventive care focuses on reducing risk and recommended screenings, while chronic disease management addresses an existing condition. Your physician can consider both within your care plan. Insurance classification of outside services depends on the service and reason for it.
Can lifestyle changes replace my medications?
Lifestyle changes can complement treatment, but they do not replace every medication. Your physician reviews whether any change is appropriate based on your condition and response. Do not stop or adjust prescribed treatment without your treating clinician’s advice.
Can you coordinate with my specialists?
We can help review recommendations, clarify your primary care plan, and coordinate with other clinicians as appropriate. Conditions that require specialist expertise continue to be managed with the relevant specialist.
Are tests, imaging, and specialist visits included?
Ask which services are included in your current membership agreement. Outside laboratory tests, imaging, specialist care, and medications may have separate charges. Coverage and costs depend on your insurer and the provider. We discuss recommended services and expected costs whenever possible.
How much does chronic disease care cost?
Our practice uses a membership-based direct primary care model. Review current membership details for fees, included visits, and exclusions. We do not bill insurance for membership-covered primary care services. Membership is not health insurance.
How do I reach the practice between visits?
Communication arrangements, appointment availability, and included services follow the current membership agreement. Review these details during your meet and greet so you know how to contact the team.
Does this practice provide emergency care?
No. Our practice supports ongoing primary care and non-emergency concerns within its scope. For a medical emergency, call 911 or go to the nearest emergency department.
