Cardiometabolic health care in Rochester
Your heart and metabolic health are closely connected. Blood pressure, cholesterol, blood sugar, body weight, kidney health, family history, and daily habits can all influence your overall risk.
At our Pittsford practice, we help you make sense of these factors together. Whether you are concerned about a recent test result, managing an existing condition, or looking to reduce future risk, your care starts with an individual assessment.
Our concierge medicine approach creates space for conversation, practical goals, and follow-up. We combine lifestyle support with medical treatment when appropriate and coordinate specialist care when needed.

Personalized cardiovascular and metabolic care
Blood pressure assessment
Review office measurements, relevant symptoms, and home readings when appropriate. Your plan may combine practical lifestyle changes, monitoring, and medication according to your needs.
Cholesterol & lipid management
Interpret cholesterol results alongside your history and other risk factors. We discuss what the results mean and whether lifestyle changes, medication, or further assessment would help.
Blood sugar & metabolic health
Assess relevant glucose or A1c results, diabetes risk, and existing treatment. We help you understand next steps and build a realistic plan for monitoring and care.
Nutrition, movement & weight
Connect medical decisions with eating patterns, activity, strength, and weight concerns. Recommendations reflect your abilities, preferences, and everyday circumstances.
Sleep, stress & tobacco use
Discuss habits and challenges that can affect your health. We identify manageable changes and appropriate evaluation or treatment resources when needed.
Risk review & care coordination
Consider family history, kidney health, medications, and other relevant factors. Further testing, imaging, or cardiology referral is selected for a clear clinical reason.
Clear answers, not a fixed package of tests
Routine measurements and appropriate laboratory tests provide a starting point. Additional tests are considered when they can help resolve uncertainty or change a treatment decision.
Advanced lipid tests, glucose monitoring, or cardiovascular imaging are not automatically necessary for every patient. Your physician explains the clinical question, potential benefits and limitations, and any expected outside costs before you decide.
We consider your overall risk rather than treating a single result in isolation. Hormone treatment is not used as a general strategy to prevent cardiovascular disease.
Your cardiometabolic care plan
Review your baseline
Bring relevant test results, medications, home readings, and questions. We review your personal and family medical history.
Choose priorities together
Identify the health measures and routines that need attention, with goals tailored to your circumstances.
Put the plan into practice
Discuss nutrition, activity, sleep, smoking cessation, and medication when appropriate. We explain the reasons behind recommendations.
Track & adjust
Follow-up reviews progress, treatment response, side effects, and whether further testing or referral is needed.
Cardiometabolic care, testing & costs
What does cardiometabolic health mean?
It describes the relationship between cardiovascular health and metabolic factors such as blood sugar, cholesterol, blood pressure, and weight. Considering these together helps your physician assess risk and plan individualized care.
Do I need care if I feel well but have abnormal results?
Some risk factors do not cause obvious symptoms. A consultation can help you understand a result in context, determine whether it needs confirmation, and decide on appropriate next steps.
Will I need advanced cholesterol tests or a coronary calcium scan?
Not everyone does. Your physician considers your history, current results, overall risk, and whether a test would change management. We explain the purpose and limitations of additional testing and discuss costs when possible.
Can lifestyle changes replace blood pressure or cholesterol medication?
Lifestyle changes can support treatment and may improve health measures, but medication can still be important. Any adjustment is made after individual assessment and monitoring. Do not stop or change a prescription without discussing it with your treating clinician.
Is a cardiometabolic assessment preventive care?
It may involve prevention, diagnosis, or management of an existing condition. Insurance classification of outside tests and services depends on what is provided and why. Our membership-covered primary care is not billed to insurance by the practice.
Are tests and imaging included in membership?
Review your agreement for included services and exclusions. Outside laboratory work, imaging, specialist visits, and medications may have separate charges. Coverage depends on your insurer and the provider. Membership is not health insurance.
Is this a cardiology clinic?
Rochester HealthSpan Medicine is a primary care practice offering individualized cardiovascular and metabolic risk assessment and management. We coordinate cardiology or other specialist evaluation when needed.
What if I have chest pain or symptoms of a stroke?
Call 911 for a suspected heart attack, stroke, or another medical emergency. This practice is not an emergency service, and routine appointments or messages should not delay urgent care.

Support beyond your test results
Your care can connect with lifestyle medicine, medical weight management, and chronic disease management. This keeps your daily routines and medical treatment working toward shared goals.
We focus on achievable progress and informed decisions. No plan can guarantee that cardiovascular disease will be prevented, and follow-up remains important as your health changes.
